Tuition to attend VIVA 10 (which includes the half-day Pre-Symposium on Monday) is:
     Physicians - $1295
     Military Physicians / Resident / Nurse / Technologist / HealthCare Professional / Industry Professional* - $850
     Monday Pre-Symposium Only - $350
     Fellows - Tuition is waived with the submission of a case study (Click here for more information)

Cancellation Policy: Cancellations are accepted until September 10 and are subject to a 15% administrative fee.
No refunds will be given after that date. VIVA must be notified in writing of your intent to cancel prior to the cancellation deadline.

Visa Letter - 8th Annual VIVA 10 Symposium Close Window

Some participants from outside the United States will need to apply for a visa at the American Embassy, consulate, or other visa-issuing office in their country of origin. Please begin your visa application process as soon as possible. Consulates in some countries take several months to process applications. For information on obtaining your visa, go to http://travel.state.gov/visa/visa_1750.html or www.dhs.gov/us-visit.

For visa purposes, a letter from the conference manager of the 8th Annual VIVA 10 Symposium may be personalized and downloaded (see below). The letter will print on VIVA letterhead and will look exactly like an original letter, except the signature will be electronic rather than signed in pen. It should be accepted as an official document; however, if it is not, please e-mail your request to the address on this page or fax it to
408-225-3240.

As soon as you contemplate travel to the United States, and after identifying that a visa is needed, contact the US embassy or consulate for information about current visa processing time estimates and processes.
